Our Social Services Department is available to assist you in coordinating needed care services. If you or your loved one is expecting a short-term stay for rehabilitation, we will work directly with you, loved ones, POA/guardian (if applicable) to plan an appropriate, safe discharge.

If long-term placement is expected, our Social Service Staff will assist you in applying for and receiving available medical assistance/support (Medicaid) and other appropriate programs, as well as assisting you or a loved one to adjust to your new home.

Amberwood Manor emphasizes the importance of not only person-centered care, but also person- directed care. This is why we strongly encourage all residents and family members to attend Care Conferences. These brief meetings with our Interdisciplinary team are focused on ensuring that we are caring for you or loved one in the manner that you desire. We are committed to our residents and their families, which is why we begin this process of collaboration upon admission to Amberwood Manor.
