Progressive Approach to Home, or PATH program, is a clinical success initiative focused on getting rehab patients home as quickly and safely as possible. In pre-discharge PATH meetings, patients and their families can expect to be involved in transitional care and discharge planning.

Additionally, Saber and the affiliated communities partner with the Patient Education Institute to use its X-Plain system, a multi-media library covering thousands of health topics. X-Plain provides excellent resources for ongoing patient and family support and education, so the discharged patients can remain healthy at home.

In the regions where Saber offers services, there are multiple affiliated nursing homes with different specialty areas. Patients and their families will be able to pick and choose the right skilled nursing facility to meet their individual needs.
