Acquired by Saber in 2020, our skilled nursing facility is located in Clarksville, VA. We’re located right by Virginia’s largest lake with direct access to Highway 58. With over 800 miles of shoreline in Clarksville, there’s plenty of outdoor scenery waiting to be explored. We strive to provide patients and residents with a relaxing and healing environment.

Expert Senior Rehabilitation and Skilled Nursing Care

Our focus on senior rehabilitation allows us to help patients and residents focus on their emotional and physical wellness. Clarksville Health and Rehab is a small, close knit Clarksville community where patients can enjoy our robust volunteer program. It’s our goal to help every patient and resident find more purpose and meaning in each day.

Clarksville Health and Rehab offers skilled nursing care, short-term senior rehabilitation and a secured memory care unit for seniors with Alzheimer’s disease or another form of dementia. We also have physical, occupational and speech therapists who provide personalized support to help each resident reach a higher level of wellness. Our skilled nursing and senior rehabilitation facility halls are designed to resemble neighborhoods to help patients and residents experience a comforting homelike feel.

Our Lighthouse Memory Care Program

Our Lighthouse Memory Care program strives to give residents with Alzheimer’s and dementia a safe and secure environment to reach a higher level of wellbeing. For example, our secured dementia neighborhood has outside access to give residents the opportunity to enjoy fresh air and nature.
