Senior Rehabilitation Center in Lynchburg, VA

Established in 1995, Forest Health and Rehab is a 97 bed skilled nursing and rehab community located in Lynchburg, VA, which many may know as “Hill City” or “The City of Seven Hills”. The facility sits on a cul-de-sac shared with Johnson’s Health Center and The Surgery Center of Lynchburg, and is conveniently located near Lynchburg General Hospital.

Each year, the community participates in The Walk to End Alzheimer’s, which they fundraise all year long for with cookouts, bake sales, yard sales, and other fun events. The team also participates in Toys for Tots every holiday season to give back to those in need.

Forest Health and Rehab is located near many shopping centers as well as local parks around town. Some attractions for visitors of the area include The James River, Amazement Square, Monument Terrace, and Point of Honor.

Forest Health and Rehab is committed to providing quality and compassionate care to the residents the community has the pleasure of serving. Whether the residents are here for short or long term care, the community hopes that they can call Forest Health and Rehab their home.

Forest Health and Rehab provides compassionate specialty services to all residents. The senior rehabilitation services include the following:

  • Respite Care
  • 24-hour Skilled Nursing Care

The 24-hour skilled nursing care service provides: cardiac care, congestive heart failure care, IV therapy, pain management, post-surgical care, pulmonary care, stroke/neurological care and wound care management.

Progressive Approach to Home (PATH)

The Progressive Approach to Home (PATH) is a unique, transitional care model that utilizes evidence-based practices and medical models. The team uses this program and transparent communication while working to return patients to their homes as quickly and safely as possible and reducing their number of hospital visits.

The community is excited to offer a Pre-registration Program that allows you to plan for your rehabilitation prior to your surgery. Community’s specialized programs are designed to enhance the lives of all patients. To help make the transition from the hospital to rehabilitation easier, the community is encouraging anyone having an elective surgery to call or make a reservation.
